Job Overview
We are seeking a dedicated and experienced Director of Care to lead our team in providing exceptional programmatic development, planning, organization and management of the functions that focus care facilitation. Case Management, Social Work, utilization Management and post-acute services
Duties
- Oversee the development and implementation of care programs tailored to meet the diverse needs of individuals with developmental disabilities.
- Supervise and support staff in delivering high-quality care services, ensuring compliance with regulatory standards.
- Manage administrative tasks related to program management, including budgeting and resource allocation.
- Cultivate relationships with community partners, families, and other stakeholders to enhance service delivery.
- Conduct regular assessments of program effectiveness and make necessary adjustments to improve outcomes.
- Provide training and professional development opportunities for staff to enhance their skills in behavioral health and social work practices.
Requirements
- Proven experience in managing care programs, preferably within the developmental disabilities or behavioral health sectors.
- Current State RN License
- Five years case management in a healthcare setting
- Strong background in social work or related fields with a focus on patient care
- Excellent supervisory skills with the ability to motivate and lead a diverse team.
- Proficient in program development, relationship management, budgeting, and administrative functions.
- Exceptional communication skills, both verbal and written, with the ability to engage effectively with clients, families, and community partners.
- A commitment to ongoing professional development and staying current with best practices in the field.
- Formal case management experience
Job Type: Full-time
Pay: $118,000.00 - $147,000.00 per year
