As a Licensed Professional Counselor (or similarly credentialed professional), you will play a pivotal role in helping patients navigate life's challenges, foster resilience, and shape effective, integrated care solutions.
Key Responsibilities :
Therapeutic Support : Provide evidence-based therapy aimed at behavior change, emotional wellness, and developing effective coping strategies. Assist patients in overcoming trauma and barriers to achieving a healthier life.
Collaboration with Care Team : Work with physicians, care managers, and primary care staff to develop and maintain holistic care plans that integrate behavioral health and medical needs.
Patient & Community Advocacy : Conduct thorough assessments to determine the best therapeutic approaches for patients. Support families and patients in accessing community resources and additional care when needed.
Crisis Management : Respond to urgent situations with professionalism, advocating for the necessary services and support for patients.
Documentation & Confidentiality : Keep comprehensive, accurate records of patient interactions and treatment in the EMR system. Ensure patient confidentiality and adhere to the highest professional standards and ethical guidelines.
What You Bring :
Qualifications : Licensed as LPC, LMSW, LMHC, or CADC with valid state credentials.
Education : Masters degree in psychology, counseling, social work, or a related field.
Experience : Preferred experience working with diverse age groups (adolescents to geriatrics). Experience in primary care or integrated health settings is a plus.
Skills & Knowledge : Strong understanding of behavioral health principles, therapeutic practices, and the impact of cultural factors on mental health. Expertise in diagnosis, treatment planning, and rehabilitation. Familiarity with electronic health records (EHR) systems.
Interpersonal Skills : Exceptional ability to connect with patients, prioritize their needs, and work collaboratively within a multidisciplinary team.
