Job Summary
Are you passionate about improving healthcare quality and ensuring patients receive the care they need? We are seeking a dedicated Quality Care Coordinator to join our team. In this role, you will focus on closing care gaps, improving medication adherence, and driving performance in HEDIS and quality metrics within a value-based care model.
As a Quality Care Coordinator, you’ll collaborate with patients, providers, and care teams to ensure preventive screenings, follow-ups, and treatments are completed. Your work will directly enhance patient outcomes and help us achieve organizational quality goals.
Duties
- Identify and manage care gaps using electronic medical records EPIC and population health tools.
- Conduct outreach to patients for preventive screenings, follow-up care, and treatment plan adherence.
- Support medication adherence initiatives by addressing barriers and collaborating with care teams.
- Monitor and improve HEDIS measures and other quality metrics, ensuring accurate documentation.
- Collaborate with providers and quality improvement teams to meet value-based care objectives.
- Maintain thorough documentation of outreach efforts, patient responses, and outcomes.
Skills Preferred
- Experience in healthcare quality improvement, HEDIS measures, or care coordination.
- Familiarity with value-based care models and population health strategies.
- Proficiency with EMRs and data management tools (e.g., EPIC, Salesforce).
- Excellent communication and problem-solving skills.
- Ability to manage multiple priorities in a fast-paced environment.
- A passion for improving patient care and achieving measurable results.
Job Type: Full-time
Pay: $23.00 - $25.00 per hour
Expected hours: 40 per week
