Key Responsibilities:
- Conduct in-home visits, health assessments, and develop care plans for patients with acute and chronic conditions
- Collaborate with interdisciplinary care teams to deliver high-quality, patient-centered care
- Participate in a shared rotation for after-hours telephonic on-call coverage
Requirements:
- Bachelor’s Degree in Nursing with an active Montana APRN license (Master’s preferred)
- Board Certification for Nurse Practitioners and DEA Licensure
- Valid Driver’s License and Proof of Auto Insurance
