What are the responsibilities and job description for the Chronic Care Manager position at My Mobile MD?
We have an opening for Chronic Care Manager, LPN, in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.
Our focus is in assisted living, memory care communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical and psych needs.
Being a mobile practice we go out to Assisted Living and Memory Care units to provide services. We usually go to 2 communities each day but with larger facilities coming on board it will be full days in each community soon enough.
My Mobile MD manages both medical and psych services. We have a full time Chronic Care Manager, LPN, covering each community and a full time NP that is usually over 2-3 communities.
This position offers a lot of flexibility and the opportunity to run your own show! Most of our LPNs are averaging $75k.
It is a very exciting practice that we have built over the past 2 years. We are the preferred provider or medical director in each community we serve. Our reputation in the industry is vastly growing with 5 more communities ready to come on board this year. All we need is you!
