Clinical Case Manager Job Description
We are looking for a dynamic clinical case manager to join our Supportive Housing Program. In this role, you'll be responsible for evaluating residents' treatment plans, monitoring residents well-being, creating case progress reports, and ensuring residents are well-informed regarding their care and treatment options.
To ensure success as a clinical case manager, you should have excellent knowledge of case management principles and the ability to act as an advocate for residents in need. Ultimately, a top-notch clinical case manager should be a compassionate, highly organized individual passionate about supporting, empowering, and rebuilding the residents in the program.
Clinical Case Manager Responsibilities:
- Handling case assignments by admitting residents to the Supportive Housing Program, reviewing records, and conducting orientation.
- Coordinating and providing resident care based on an approved treatment plan.
- Facilitating aspects of the treatment plan, such as care coordination and discussions with the family of the resident.
- Recording all case information, such as care procedures, appointments, and check-ups.
- Arranging for the intervention of social services, health, or governmental agencies when needed.
- Determining the need for therapeutic, psycho-social, or psychiatric evaluations based on resident progress.
- Interacting with residents to ensure they are informed about their treatment plan, and answering any questions they may have.
- Evaluating residents' progress and writing case reports to determine case closure.
- Building strong relationships with the supportive housing and external clinical team throughout the case management process.
- Assessing residents’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans.
- Working with residents, family and friend support networks and health care professionals to put care plans in place.
- Listening to residents’ concerns and providing counseling or intervention as required
- Recording residents' progress including charting referrals, home visits and other notable interactions
- Evaluating resident’s progress periodically and making adjustments as needed to improve outcomes.
- Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mental health.
Clinical Case Manager Requirements:
- A master’s degree in social work, or a relevant healthcare field.
- A minimum of 3 year's experience as a clinical case manager, therapist, clinician or a similar healthcare role.
- Excellent knowledge of case management principles and healthcare management.
- Ability to work with residents with disabilities, mental illnesses, or challenging behaviors.
- Good communication and interpersonal skills.
- Ability to be compassionate, empathetic, resident, and calm when working with residents.
- Solid organizational skills and a high level of attention to detail.
- Ability to remain up-to-date with emerging medical knowledge and technology.
Job Type: Full-time
Pay: $22.00 - $26.00 per hour
Expected hours: 40 per week
