Job Summary
The Care Coordinator will serve as the primary liaison between patients, providers, and other medical staff to ensure timely and effective care coordination. Key Responsibilities:
- Greet patients upon arrival, offering a welcoming and supportive environment.
- Accompany patients during various points of their visit to ensure they understand their care plan, procedures, and next steps.
- Serve as the go-to support for patients during visits—answering questions, explaining procedures, and helping with paperwork or navigation within the clinic.
- Collaborate with the ENT provider during visits to reinforce instructions and treatment plans in patient-friendly language.
- Coordinate and schedule follow-up appointments, diagnostic tests, imaging, and specialist referrals during or immediately after patient visits.
- Obtain necessary insurance pre-authorizations and verify coverage prior to procedures or follow-ups, keeping patients informed.
- Provide hands-on assistance with post-visit instructions, medication guidance, and care coordination.
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR) system.
- Ensure that test results and follow-up actions are tracked and communicated to patients and providers promptly.
- Support front office functions, including check-in/check-out and phone coverage, as needed.
Skills/Abilities:
- Excellent organizational and multitasking abilities.
- Proficient in medical office software and electronic health records (EHR).
- Effective written and verbal communication skills.
- Problem-solving and analytical skills.
Education/Experience:
- High school diploma or GED required; Associate’s or Bachelor’s degree in a related field preferred.
- Minimum of 2 years in a medical office setting, with patient experience.
Physical Requirements:
- Prolonged periods sitting at a desk and working on a computer.
- Prolonged periods of standing and bending.
- Must be able to lift up to 15 pounds at times.
