What are the responsibilities and job description for the Licensed Certified Pharmacy Technician position at MyCareer+?
We are looking for a Certified or Licensed Pharmacy Technician to join our clients team in a primary care setting.The ideal candidate will play a key role in assisting pharmacists with medication preparation and distribution maintaining patient records and delivering exceptional customer service.
Key Responsibilities :
Review and verify patients medication histories to ensure accurate documentation.
Identify and resolve discrepancies in medication records working closely with pharmacists to make necessary corrections.
Educate patients on their medications including proper usage potential side effects and the importance of adherence.
Address patient inquiries or refer them to the appropriate healthcare professional for complex issues.
Monitor patients medication adherence through electronic health records (EHR) and other tracking systems.
Follow up with patients at risk of nonadherence communicating concerns to the care team.
Facilitate communication between healthcare providers to ensure seamless care transitions particularly after hospital discharges or care changes.
Assist with medication refills prior authorizations and delivery coordination.
Accurately update patient data in the EHR system.
Document all patient interactions interventions and communications with healthcare providers.
Aid pharmacists in conducting thorough medication reviews and identifying potential drug interactions or contraindications.
Support the care management team with data on medication usage and contribute to care plan development.
Ensure compliance with HIPAA state pharmacy laws and other relevant regulations.
Participate in quality improvement initiatives aimed at enhancing patient care and safety.
