We are looking for a compassionate Licensed Master Social Worker (LMSW) to join our team and provide behavioral health support to patients while collaborating with our multidisciplinary care team.
What You'll Do :
Therapeutic Interventions : Deliver evidence-based therapy, focusing on emotional health, coping strategies, and behavior modification. Help patients process trauma and build resilience.
Integrated Care Collaboration : Work with physicians, care managers, and primary care teams to develop and maintain comprehensive care plans. Act as a behavioral health expert, offering valuable insights on patient needs.
Patient and Community Support : Conduct screenings, provide assessments, and guide patients and families in accessing community resources. Advocate for patients to ensure they receive the services they need.
Documentation and Confidentiality : Keep accurate, up-to-date records in the EMR, ensuring confidentiality and compliance with ethical standards.
Qualifications :
Masters degree in Social Work (MSW).
Active LMSW license in Michigan.
Experience in behavioral health care and crisis intervention.
Strong communication and teamwork abilities.
