Key Responsibilities : Therapeutic Care : Deliver personalized, evidence-based therapy that focuses on emotional wellness, behavior management, and stress reduction. Help patients process past trauma and develop coping mechanisms for a healthier future. Collaborative Care : Work closely with a diverse team of healthcare professionals, including physicians and care managers, to create and implement holistic, patient-centered treatment plans. Community and Resource Support : Conduct comprehensive assessments to identify appropriate therapeutic strategies, and connect patients and families with community services, resources, and referrals when needed. Crisis Intervention : Provide immediate support in high-stress situations, helping patients navigate crises and ensure they receive the care they need. Documentation & Compliance : Maintain precise and thorough records of patient progress and interventions, ensuring strict adherence to privacy standards and professional ethics.
What You'll Bring : Licensure : Current state licensure as LPC, LMSW, LMHC, or CADC. Education : Masters degree in counseling, social work, psychology, or a related field. Experience : Experience working with individuals of all ages, from adolescents to older adults, is preferred. A background in integrated health or primary care settings is a plus.
Skills : A deep understanding of therapeutic techniques, mental health assessments, and cultural sensitivity. Expertise in treatment planning, behavioral health interventions, and managing complex cases. Technological Proficiency : Comfortable using electronic health records (EHR) systems to document and track patient care. Compassionate Communication : Strong interpersonal skills with the ability to create a trusting, supportive environment for patients and their families.
