Responsibilities
Assist in the review and verification of patients' medication histories to ensure accurate recording.
Identify and resolve discrepancies in medication records, collaborating closely with pharmacists.
Educate patients on prescribed medications, covering proper usage, potential side effects, and the importance of adherence.
Address patient inquiries or direct them to appropriate healthcare professionals for complex concerns.
Monitor and evaluate patients' adherence to medication using electronic health records (EHR) and other monitoring tools.
Follow up with patients who miss doses or show signs of non-adherence, communicating concerns to the care team.
Facilitate communication among healthcare providers to ensure seamless transitions of care, especially during hospital discharges or changes in care settings.
Assist in managing medication refills, prior authorizations, and medication delivery services.
Accurately update and maintain patient information in the EHR system.
Document all patient interactions, interventions, and communications with healthcare providers.
Support pharmacists in conducting thorough medication reviews and identifying potential drug interactions or contraindications.
Provide data on medication usage to the care management team and contribute to care plan development.
Ensure compliance with HIPAA and state pharmacy laws in all activities.
