Job Description

Job Title : Occupational Therapist

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : 8 : 30 AM - 5 : 00 PM

Guaranteed Hours : 40 hours per week

Experience : Minimum one year of Home Care experience

License : Licensed Occupational Therapist

Certifications : BLS (AHA)

Must-Have :

Active Driver's License
Auto Insurance
Reliable Vehicle
Clean DMV Report
Experience in Outpatient, Ortho, Neuro, and Pediatrics
Skills Checklist pertinent to the specialty
Supervisory Reference Forms from the last two years

Job Description :

Evaluate patients' function through formal and informal testing to determine various status levels including skeletal, neurological, respiratory, cardiovascular, developmental, social, and cognitive functions.

Develop a goal-specific treatment plan based on assessment data and patients' tolerance to carry out the treatment regimen as prescribed.

Submit a written treatment plan to be included in the plan of treatment authorized and coordinated with the physician. Update the plan of care as needed to reflect changes in patient status.

Devise a treatment schedule, including frequency and duration, to be discussed with patients, family, and caregiver, and coordinated with clinical support staff.

Educate patients, families, and caregivers regarding condition pathology, alternatives, prognosis, and rationale for treatment through various forms of rest, exercise, modalities, and other indicated interventions.

You will be required to work one weekend per month rotation.
