\n- Provide Personalized Nutrition Support: Evaluate and address the unique nutritional needs of each participant, creating individualized care plans that include nutrition-focused physical exams, food insecurity assessments, and recommendations for nutritional supplements.
- Collaborate with a Supportive Team: Be an active and valued member of the interdisciplinary team (IDT), working together to coordinate and deliver holistic, participant-centered care.
- Monitor Participant Wellness: Keep a caring eye on participants’ health by monitoring weight, hydration, dietary intake, and changes in chewing, swallowing, mood, or medical conditions. Share updates with the team to ensure proactive care.
- Ensure Meals Meet Participants' Needs: Oversee menu planning and meal delivery to ensure participants receive delicious, safe, and medically tailored meals that meet their dietary requirements.
- Uphold Quality in Food Services: Partner with the Nutrition Supervisor to maintain high standards in food services, supporting best practices and compliance with health codes and PACE regulations.
- Step In When Needed: Occasionally step in to support the Food Services Coordinator, helping to oversee meal services and ensure everything runs smoothly.
- Conduct Meaningful Assessments: Visit participants in person at the center or, when needed, in their homes to conduct annual or semi-annual nutrition assessments. Provide compassionate follow-up education and counseling to help participants live healthier, more fulfilling lives.
- Inspire Through Education: Create and lead engaging nutrition education programs, such as wellness classes, cooking demonstrations, and informative handouts, to empower participants and their families.
- Train and Empower Staff: Provide regular training to day center staff on food service best practices, fostering a culture of safety, quality, and participant care.
- Ensure Program Compliance: Review menus and other program components to meet the requirements of the Child and Adult Care Food Program (CACFP), ensuring participants’ nutritional needs are met with care.
- Support at Home: Coordinate and document the delivery of oral nutrition supplements and home-delivered meals, ensuring participants feel supported both at the center and in their homes.
- Document with Care: Maintain accurate and timely documentation of participant interactions and care plans in the EMR, reflecting the quality and thoughtfulness of the care provided.

- Education & Licensure: A Bachelor’s degree in Nutrition, Dietetics, or a related field is required. Must hold an active Registered Dietitian (RD) license and be in good standing with the Academy of Nutrition and Dietetics. Certification in CPR/BLS is required or must be obtained within 30 days of hire.
- Passion for Serving Older Adults: Demonstrated dedication to improving the quality of life for high-risk seniors and frail older adults. Previous experience in a PACE program, long-term care, or similar setting is highly preferred.
- Strong Clinical Expertise: Proven ability to evaluate and address nutritional needs, develop care plans, and conduct nutrition-focused physical exams with precision and care.
- Collaborative Mindset: Ability to work effectively as part of an interdisciplinary team (IDT), contributing to a supportive, participant-centered care environment.
- Attention to Detail: Experience in monitoring participants’ health, identifying changes in diet and health status, and ensuring timely and accurate documentation in the EMR.
- Food Service Knowledge: Familiarity with menu planning, meal preparation, and adherence to safety and regulatory requirements, including CACFP standards and local health codes.
- Adaptability & Leadership: Willingness to step in and support food service operations when needed, including overseeing meal services and training staff on best practices.
- Education & Counseling Skills: Proficiency in providing nutrition education, counseling, and creating engaging educational programs such as wellness classes, cooking demonstrations, and participant handouts.
- Organizational Skills: Experience managing multiple tasks, such as coordinating the delivery of oral nutrition supplements and home-delivered meals, and maintaining compliance with program regulations.

- We’re excited to share that we plan to hire for this role in February/March, with our new site opening in July 2025! Onsite training will be provided at our Greater Los Angeles location to set you up for success.

Salary : $81,200 - $94,200
