About this Role
This is an opportunity for a mission-driven team player who has experience managing home care services to develop and implement home care services for PACE program participants, including the coordination of all Durable Medical Equipment and community-based care. You are a collaborative team member who works as part of the Interdisciplinary Team to create a care plan to help participants’ meet their goals. This is primarily a center-based role.
\n- Coordinating Home Care Services - Collaborate with trusted home care agencies to ensure our participants receive personalized, compassionate care that meets their unique needs.
- Contributing as a Key Team Member - Be an integral part of our Interdisciplinary Team (IDT), conducting home assessments, creating thoughtful care plans, and gathering vital information to guide the best possible services for our participants.
- Providing Compassionate Nursing Care - Deliver dedicated nursing care within the LVN scope, whether in the clinic or in participants' homes, with a focus on comfort and well-being.
- Documenting with Purpose - Maintain clear, accurate, and timely records in the Electronic Medical Record (EMR) system, ensuring seamless communication and continuity of care for all participants.
- Collaborating for Participant Success - Work closely with home care employees to monitor participants’ health, ensuring any changes are promptly communicated to the RN and Physician for ongoing, quality care.
- Supporting Smooth Transportation - Partner with the Transportation Coordinator to make sure participants can easily get to their appointments, removing any barriers to care.
- Managing Appointments - Assist with scheduling home care visits and provide backup support for other appointment scheduling as needed, ensuring no gaps in care.
- Ensuring Quality Care - Oversee visits from external agencies to ensure that all services provided meet the highest standards of quality and truly benefit the participants’ health and well-being.
- Be a Team Member – Step in when necessary to assist the care team, fostering a positive and efficient atmosphere that benefits both colleagues and participants alike.
- Experienced and Knowledgeable: You have a current LVN license and have experience in providing nursing care in both clinic and home care settings. Your understanding of the LVN scope of practice allows you to deliver high-quality, compassionate care to participants.
- Person-Centered Approach: You’re deeply committed to tailoring care to each participant’s individual needs. You have a passion for enhancing the lives of those you serve, providing care with empathy, patience, and respect.
- Team-Oriented: You work effectively as part of a multidisciplinary team, collaborating with physicians, nurses, and other healthcare professionals. You actively contribute to care plans, assessments, and communication, always with the goal of improving participant outcomes.
- Excellent Communication Skills: You excel in both written and verbal communication. You document accurately in EMR systems, and you’re skilled at conveying important health information to team members and participants' families.
- Adaptable and Flexible: You’re comfortable working in dynamic environments, whether in participants' homes, in the clinic, or in collaboration with outside agencies. You can adjust to varying needs and priorities with a positive attitude.
- Organized and Efficient: You keep accurate records, stay on top of appointments, and manage your time efficiently to ensure care is delivered on schedule. Your attention to detail ensures that nothing falls through the cracks.
- Compassionate and Caring: Your kindness and genuine care shine through in your interactions with participants. You go beyond just the physical aspects of care, offering emotional support and a sense of security to those you serve.
- Up to 80% of your role will involve travel to participants' homes
- Schedule is Monday - Friday 8a-5p
- Office Setting: Work is performed in a professional office environment within a healthcare facility, using computers, phones, and scheduling software.
- Seated & Active Work: The role involves extended periods of sitting at a desk, typing, and speaking on the phone, along with occasional movement throughout the center to collaborate with staff.
- Fast-Paced & Team-Oriented: You’ll work in a busy and engaging environment, balancing multiple priorities while ensuring participants receive seamless care.
- Noise Level & Distractions: The office environment may have moderate noise due to phone calls, conversations, and activity in the center.
- Technology Use: Regular use of computers, scheduling software, and EMR systems to document and coordinate participant care.
