Job Description:
As a Nurse Navigator at Myrtue Medical Center, you will play a critical role in ensuring seamless patient care across the healthcare continuum. This includes coordinating communication and collaboration between internal and external clinical staff, providers, and patients.
Responsibilities:
- Care Coordination: Provide comprehensive coordination of patient care from preadmission to discharge, ensuring continuity and quality service
- Point of Contact: Serve as the first point of contact for patients and families, addressing their needs and concerns in a timely and compassionate manner
- Collaboration: Work closely with internal and external clinical staff to ensure effective communication and collaboration
- Professional Development: Stay up-to-date with current issues and ideas in nursing practice, applying best practices to enhance patient care
Requirements:
- Qualifications: RN or LPN licensure; two years' experience in clinical nursing required; BSN preferred
- Skillset: Previous case management and/or nurse navigator experience preferred; working knowledge of health insurance beneficial
