About Us: Mystis Adult and Family Services is dedicated to providing comprehensive care and support to individuals experiencing homelessness. Our goal is to assist clients in transitioning to stable housing while addressing their immediate health and social needs.
Position Overview: We are seeking motivated and compassionate Case Managers to join our team in a hybrid role, working both in the community and at our recuperative care facility in Salinas as well as from home. The Case Manager will provide direct support to clients, helping them navigate services, secure housing, and achieve their recovery goals. This position requires strong interpersonal skills, flexibility, and the ability to work independently and as part of a team.
Key Responsibilities:
- Client Support and Advocacy:
- Conduct comprehensive assessments of client needs and develop individualized care plans.
- Assist clients in accessing healthcare, social services, and housing resources.
- Provide case management services, including goal setting, progress monitoring, and advocacy.
- Conduct home visits and community outreach to support clients in various settings.
- Housing Assistance:
- Guide clients through the housing application process and assist with securing permanent housing.
- Collaborate with housing providers, landlords, and other stakeholders to facilitate housing placements.
- Support clients in maintaining housing stability through follow-up visits and ongoing support.
- Health and Wellness:
- Monitor clients' health status and ensure they receive appropriate medical care and follow-up.
- Provide basic health assessments and first aid as needed.
- Educate clients on health and wellness practices, including medication management and nutrition.
- Facility-Based Support:
- Work at the recuperative care facility to provide on-site case management and support.
- Coordinate care with facility staff to ensure a seamless continuum of services.
- Assist with daily activities and address any immediate needs of clients at the facility.
- Documentation and Reporting:
- Maintain accurate and up-to-date client records, case notes, and reports.
- Ensure compliance with HIPAA and other confidentiality standards.
- Participate in team meetings and contribute to care planning and case reviews.
- Community Collaboration:
- Build and maintain relationships with community organizations, service providers, and local agencies.
- Represent Mystis Adult and Family Services at community meetings and events.
- Advocate for clients' needs and collaborate with partners to address service gaps.
Qualifications:
- Bachelor's degree in social work, psychology, public health, or a related field (or equivalent experience). Preferred. Lived Experience welcome!
- Previous experience in case management, social services, or a related field.
- Strong understanding of homelessness, housing resources, and community services.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and manage a diverse caseload.
- Valid driver's license and reliable transportation for community-based work.
- Proficiency in Microsoft Office and case management software.
Preferred Qualifications:
- Lived experience or prior work experience in a similar setting.
- Familiarity with motivational interviewing and trauma-informed care practices.
- Bilingual skills (English/Spanish) are a plus.
Work Environment:
- This is a hybrid role, requiring both community-based work and on-site support at the recuperative care facility.
- Exposure to various health conditions and illnesses is possible; appropriate precautions and PPE will be provided.
- Flexibility in working hours may be required to meet clients' needs.
