Position Summary

The Licensed Vocational Nurse (LVN) provides direct patient care using the nursing process under appropriate supervision. The LVN performs focused assessments, assists in care planning and interventions, administers medications, and documents patient conditions in compliance with the Texas Nursing Practice Act (NPA). This role requires strong communication, decision-making, and clinical skills to ensure the highest quality of care for adult and geriatric patients in the Geri-Psych unit.

Essential Duties & Responsibilities

Patient Care & Nursing Duties

• Collect data and perform focused nursing assessments on patients.
• Assist in the planning, development, and modification of nursing care plans.
• Provide health education and counseling to patients and families to promote wellness.
• Assist in evaluating patient responses to interventions and identifying patient needs.
• Administer medications and treatments safely, ensuring proper documentation.
• Recognize and report adverse reactions to medications, treatments, and procedures.
• Clarify any questionable medication orders with the prescribing provider.
• Implement measures to maintain a safe environment for patients and staff.

Clinical Documentation & Communication

• Accurately document patient status, nursing care, physician orders, and administered treatments.
• Communicate clearly and promptly with the healthcare team regarding patient conditions.
• Maintain patient confidentiality and comply with HIPAA regulations.
• Ensure proper documentation accompanies patients when transferring for procedures.

Regulatory & Compliance Responsibilities

• Follow all nursing practice standards outlined by the Texas Nursing Practice Act.
• Report any patient safety concerns, medication errors, or professional misconduct.
• Adhere to infection control protocols, including proper PPE use.
• Maintain professional boundaries and provide care without discrimination.

Professional Development & Collaboration

• Attend department meetings and complete all required training and competency evaluations.
• Stay current with nursing best practices, procedures, and new healthcare technologies.
• Collaborate with interdisciplinary teams to provide optimal patient care.
• Mentor and support other nursing staff as needed.

Qualifications & Requirements

Education & Experience

• Required: Current Texas LVN license in good standing.
• Required: BLS (CPR) certification.
• Preferred: Experience in emergency or psychiatric care settings.

Skills & Competencies

Physical & Work Environment Requirements

Physical Demands

• Standing/walking for extended periods (61-100% of shift).
• Frequent bending, squatting, lifting (up to 50 lbs), pushing, and pulling.
• Occasional driving and patient transport duties.
• Use of hands for gross and fine motor tasks.

Work Environment

• Indoor hospital setting with exposure to airborne and bloodborne pathogens.
• Potential exposure to hazardous chemicals and sharp objects.
• Required adherence to infection control and safety protocols.
