NAFI Connecticut Inc is a multi-service agency offering a wide range of residential, programming consultation, and personalized support to youth, families, and adults across Connecticut, New York, and Rhode Island. With unique services designed for each state, we provide trauma-informed care and community-based youth programs in Connecticut, comprehensive family support and residential services for at-risk youth in New York, and targeted mental health services and crisis intervention in Rhode Island and so much more. NAFI New York is exploring candidates for our Family Wrap Around program seeking a compassionate, dedicated Therapist / Clinician to provide clinical services to families in Orange County. The Family Wrap Around Clinician has a pivotal role in providing clinical services via individual and family sessions to address family challenges, assist family with achieving goals and support safety and well-being children and families. To engage and effectively work with families within their natural environment the Clinician provides services to the families in their home and the community. The clinician is primary service provider supporting families and ensuring safety and well-being.
Responsibilities Include :
- Complete intake screenings of new referrals in accordance to NAFI NY and Family Wrap Around standards
- Conduct clinical assessment and recommendations
- Provide Clinical intervention services to families in accordance with Family Wrap Around Program Model
- Provide support and guidance to families to achieve service plan and treatment plan goals
- Provide linkages to community services to the family to ensure that they receive services needed
- Maintain a caseload of up to 8 families to provide interim clinical services, while also offering consultation and support while community linkages are established
- Provide services to families at times convenient to them including evenings and weekends
- Conduct minimum required visits with families each month as per NAFI NY, DSS and OCFS requirements
- Collaborate with Department of Social Services on family's needs and services being provided
- Team approach to connect families with service providers to support families achieving goals
- Provide on call support evenings and weekends to assist with crisis management, intervention and de-escalation when needed
- Document progress notes, reports, treatment / service plan that demonstrate the case management support provided to families including success and challenges of the family progress towards goal attainment.
- Report both written and orally to stakeholders and courts on the progress and barriers the family is experiencing

Salary rates : $62,000.00 - $64,000.00 / per year - Dependent on skills, education and experience
