NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. We believe that people can change, and we work to empower each individual in their own care.
We are currently exploring candidates for Life Coach/ Peer Support Professional MST-EA to provide case management, teach life skills, and mentor 17-21 year olds participating in an intensive community-based program.
Coach/Peer Support Responsibilities:
- Serve as a coach/peer support to young adults with a mental health diagnosis and DCF involvement in conjunction with the clinical supervisor and therapist
- Engage in prosocial activities with young adults in the community
- Engage in mentoring activities
- Teach life skills to help the young adult meet their vocational, educational, and social goals
- Must be flexible to meet the needs of clients
- Participate in weekly supervision to identify activities and curriculum
- Complete electronic documentation after each session with a client
- Must enjoy working as part of a team
- Must be willing to travel- drive to and from clients homes, drive clients around
- Complete mileage forms to track car mileage
- Code in/out of sessions with a supervisor
- Service caseload of up to 4 clients
- Weekend availably preferred
- Laptop, cell phone, and mileage reimbursement provided.
Coach/Peer Support Qualifications:
- Bachelor’s degree in human services or related field
- Lived experience welcomed- knowledgeable of the issues that affect at-risk youth
- Previous experience working with youth/young adults
- Comfort working with individuals with mental health and substance use conditions
- Comfort working with individuals with behavioral challenges
- Comfortable driving to and from clients homes, and driving them around to different activities or to accomplish tasks
- Have a reliable car, license, and active car insurance
- Basic computer skills (ie: Microsoft word, Microsoft Outlook, electronic health record, etc)
Hourly: $20.00 / per hour (number of hours vary and are dependent on number of clients assigned)
