Case Manager
NAMI Southeast Louisiana is hiring a case manager for our Cornerstone Housing Program.

About you: You see the potential in others and find it rewarding to support persons with mental illness, assisting them with finding the resources and natural supports that will allow them to reach their goals and feel part of their community. You recognize that each individual has their own unique strengths, abilities, needs and preferences. You believe in the Housing First Model for addressing homelessness that prioritizes access to permanent, stable housing, linked with voluntary services as needed. Housing First recognizes that stable housing is a prerequisite for effective psychiatric and substance abuse treatment and for improving quality of life.
Core duties and responsibilities: Meet regularly with program clients to assist them with reaching goals as outlined in individualized treatment plan developed by the team clinician and client. Work with recently homeless individuals with vouchers to secure permanent housing, benefits, psychiatric and medical services. Assist clients in identifying triggers, natural supports, coping strategies, and strengthening their resilience. Provide supportive counseling. Document services in electronic health record. Participate in scheduled team meetings and consult regularly with team clinician. Assist in locating housing units, completing application process, and maintaining tenancy.
Education required: Bachelor’s or Master’s in Psychology, Sociology, Rehabilitation, or Criminal Justice . Master’s in counseling or social work is preferred. Supervision for individuals seeking LPC or LCSW licensure is available.
Licensure required: CSW, LMSW, or PLPC is preferred but not necessary.
