Job Title : Admissions Coordinator
Facility : Nassau Rehabilitation and Nursing Center
Location : Hempstead, New York
Shift : Full-Time, 9am - 5pm
Pay Rate : $22 per hour
Responsibilities :
- Cover Admissions Department in the absence of the Admissions Director
- Assist the Admissions Director in all required Admissions department functions
- Attend Morning Report if needed by Director / Administrator
- Complete daily AM census & End of Day (EOD) reports
- Communicate all Admissions-related information to the interdisciplinary team (Admissions alert, chats, lists)
- Complete Admissions Agreements for new admissions
- Maintain ongoing audits for 100% compliance with PRI, screens, Admissions Agreements, and PRI log
- Manage referral sources (Allscripts, Navi, faxes) with a response time of 90% within 15-30 minutes
- Perform data entry and upload into Sigma and billing systems before new admission
- Collaborate with the marketing / outreach team and hospitals to maximize potential admissions and census growth
- Communicate with families to assist in converting referrals while promoting facility highlights
- Maintain up-to-date knowledge of facility clinical capabilities, policies, and marketing points
- Understand insurance, payor sources, disenrollments, and SNF finances
- Answer calls and assist with directing, insurance questions, and general inquiries
- Maintain hospitalization list daily and communicate to outreach and social work teams
- Assist in preparing for admissions, including chart preparation, room arrangements, and equipment coordination
- Ensure rooms are ready for admissions, collaborating with nursing, housekeeping, and concierge
- Help outreach team with preparing literature, giveaways, and managing promo item inventory
- Send out email admission alerts
- Obtain financial and relevant information during intake and admissions
- Assist with family tours and attend meetings when necessary or in the absence of the Admissions Director
- Report departmental concerns or observations to Admissions Director
- Perform administrative tasks as needed
- Available off business hours if necessary
