Case Manager/Social Worker
Job Purpose: Provides case management for clients of Natick Service Council, Inc. (NSC). NSC is a nonprofit agency whose mission is to connect, engage, and collaborate with the community to support Natick neighbors who are experiencing hardship or economic uncertainty by promoting self-sufficiency. The Case Manager position is part-time and reports to the Director of Case Management.
Duties and Responsibilities:
- Conducts intakes with new clients of the Natick Service Council
- Supports existing clients to help identify and overcome barriers towards self–sufficiency through utilization of NSC programs and referrals/advocacy to other local services programs
- Works with clients to identify and prioritize service needs and develop alternative strategies for meeting those needs
- Assists clients with completing forms and accessing resources
- Advocates on client’s behalf with agencies, funding sources, landlords, utility companies, etc. as necessary
- Initiates and maintains close working relationships with local resources, engages in information-sharing and mutual consultation and participates in joint problem solving
- Makes financial assistance and/or advocacy recommendations during team case management meetings
- Collaborates with team on executing NSC programs for clients
- Other duties as assigned
Qualifications:
- Bachelor’s degree from accredited college, Master’s degree in social work preferred
- Minimum of two years of working with low-income population preferred
- Experience in case management
- Knowledge of local resources
- Ability to work with and understand problems and outlooks of low income population
- Good organizational, communication (oral and written), planning and creative thinking skills
- Ability to work well with others and as part of team
- Bilingual or multilingual (Spanish, Portuguese) proficiency preferred
- Experience with Salesforce database, Gmail, and Google Drive preferred
Special Requirements:
- Schedule: Part-Time 20-30 hours/week in office (NSC is open Monday through Thursday: 8am to 5pm)
- Available to work occasional evening or weekend hours for NSC programs and/or events
- Able to multitask and prioritize task completion
