Compensation:
Orientation: $20
Piece rate: $33 - $34
Location: Charlotte, NC
We are seeking a full-time Intensive In-Home specialist to provide the following services:
- Serve as a member of the client’s clinical treatment team and create, develop, monitor, revise, and implement the client’s Person-Centered Plan.
- Participate in the development of crisis plans and transition plans when clients move in and out of services.
- Provide individual and family focused support to clients and their families/natural supports based on their needs, diagnoses and best evidence-based practices.
- Provide any written documentation as required. This will include treatment notes, goals and plans for the medical record (e.g., Service notes, person-centered plan etc.)
- Implement evidence based practice selected by agency.
- Must be free from all illicit substance use.
Direct Services
- Work to implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities and other best evidence-based practice therapies.
- Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan.
- Contribute to and participate in child and family team meetings.
- Provide parent/guardian/natural support training that will aid persons interacting with the client in dealing more effectively with the client.
- Assist in all service delivery as formally planned with the clinical team.
This position will be mostly in the community and in clients homes, and may require some evening work.
