1. Support implementation of EHR/EMR system
2. Conduct research, mapping community assets, or reviewing data to strengthen the ability to meet community needs
3. Coordinate amongst patient support services and care teams
4. Communicate with stakeholders
5. Create tracking or data management systems
Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements.
Skill with Microsoft Office.
Must speak Spanish.
