Position Summary
The member will serve at Trinity Child Care Center, Scranton, PA. The program is licensed through the Pennsylvania Department of Health. Trinity provides childcare on a daily for up to 60 children who have medically complex conditions.
The member will serve in enhancing the continuity of care that surrounds the child during their developmental years. The children live with their families throughout Lackawanna and surrounding counties.
The CHW as a patient navigator will help to ensure care coordination through exploring needed resources, advocacy and communication with families, school, relevant health providers and others.
Program location is open 6 am to 6 pm. There is flexibility for the member to make up hours during the service year.
The shift will be Monday through Friday, 8 am to 4 pm with a 30 minute lunch break that does not count towards member's service hours. The service member will serve a 38 hour week.
Major Duties and Responsibilities
The major duties and responsibilities of the Pediatric Public Health Family Navigator is to advocate for quality medical care for individuals who receive services through Trinity Child Care Center. In addition, the individual in this position will advocate for meaningful interactions and opportunities within the community. This is done through the preparation for appointments, attendance at appointments, attending and providing accurate information to the medical provider and then relaying this information back to the appropriate parties at the Trinity.
In addition, the Navigator is responsible for assisting in care coordination and resource navigation. This might include family contact and cross system communication. The Health Navigator will also explore meaningful opportunities to improve or expand individual’s mental, social and emotional health by investigating resources within the community that enhance an individual’s quality of life. Maintaining current information about available resources would assist current and future children, their families and the staff who interact with them.

Good grooming and appropriate dress reflect employee pride and inspire confidence. Employees are expected to wear attire that is suited to the duties they provide. Various departments have their own expectations for work attire. Refer to your supervisor for department specific policy. Direct service employees and frontline managers are expected to wear clothing that is comfortable, casual and suited to the activities planned for that day and weather conditions. Management and office employees are expected to dress in more business-like attire and in accordance to their overall role and determined by their day’s activities.
St. Joseph’s Center identification badges are also considered part of work attire and are expected to be worn by each employee during work hours.
Inappropriate work attire, not permitted to be worn, includes, but is not limited to the following:
Shoes that are worn at work should be secure and safe for the routine activities of one’s position. Shoes should also be secure and safe for participation in unplanned activities such as using of steps, responding to a fire drill, etc.
Staff working in a direct care or direct care supervisory position should wear shoes with closed toes and back.
Staff driving a Saint Joseph’s Center vehicle should wear shoes that are secure with a strap.
