DEPARTMENT: NURSING SERVICES
1ST LINE SUPERVISOR: DIRECTOR OF NURSING OR DESIGNEE
WORK AREA: ENTIRE FACILITY OR AREA AS ASSIGNED
PURPOSE: The purpose of this position is to follow facility policies and procedures to implement treatment protocol; review and recommend resident treatment needs and take appropriate action and to participate in oversight of care provided to residents
WORKING HOURS:
80 hours per pay period plus or minus as business needs and assigned by the Director of Nursing or designee.
THIS PERSON REPORTS TO: Director of Nursing or Designee
ESSENTIAL FUNCTIONS:

NON-ESSENTIAL FUNCTIONS:
Other duties as assigned.
WORKING CONDITIONS: Works in well-lit, well-ventilated nursing units and rooms. May stand, stoop, bend and walk intermittently during the workday. May have to deal with emotional disturbances of residents, residents family and staff. May be subject to falls, burns from equipment, scratches, and kicks from confused residents. May be exposed to infectious diseases and odors, and personality peculiarities of the aged. Required to follow the No Lift Policy and must be on constant alert to reduce risk of resident injuries.
HIPAA MINIMUM NECESSARY: Responsible to adhere to HIPAA MINIMUM NECESSARY guidelines and safeguard protected health information.
DUITES AND RESPONSIBILITIES:
1. Clock in and out promptly as assigned.
2. Communicate daily with CNAs, charge nurses, and supervisors related to resident skin condition, change in treatment orders, preventive skin care needs, etc. as indicated.
3. Follow physicians orders to carry out all treatments as ordered for entire facility (ointments, dressing changes, etc.).
4. Documents each treatment as it is completed.
5. Assures that treatment orders are written and entered in the EMR per physicians orders.
6. Initiates/assures initiation of appropriate skin sheet for all skin areas in a timely manner (as soon as noted and initial treatment is begun):
- Pressure ulcers (for all pressure ulcers) pink sheet
- Skin Monitor Sheet (for all skin areas other than a pressure ulcer) white sheet
- Note: each area to be entered on a separate skin sheet as appropriate.
7. Obtains and processes physicians orders as needed.
8. Enters new skin areas (of any type) on the acute board FYI status.
9. Follows White Oak Management, Inc. wound care protocol.
10. Documents measurements and descriptions of all skin areas at least every 7 days on skin sheet.
11. Collaborates with RN regarding staging and/or classification of skin areas.
12. Collaborates with RN regarding effectiveness of treatments every 14-30 days if no change in status of affected area (reviews with DON prior to contacting physician for change in treatment orders).
13. Responsible for performing daily body checks of residents with actual pressure areas and weekly checks for residents with no actual pressure areas, as indicated per skin risk score.
14. Complete/prepare weekly Pressure Ulcer Report turn in to DON and scan to primary nurse consultant each week.
15. Assists unit nurse with body audit of residents on admission or readmission to the facility when on duty (completion of form N074).
16. Maintain the treatment carts, supplies, and all solutions, ointments, etc. Assure that all containers are properly labeled, dated when opened, and removed after treatment is discontinued.
17. Reviews form N074 on next scheduled working day, if not on duty when resident admitted.
18. Completes Skin Risk Data Collection Tool (UDA), on admission and weekly times four (4) after admission, readmission and each ninety (90) days, with the care plan schedule, thereafter.
19. Collaborate with RAC nurse for coding of MDS skin section.
20. Collaborates as a team member in the development of resident care plans as assigned, and assists in implementing the plan on the units.
21. Assists in providing direct resident care (i.e., personal care, bath, elimination needs, OOB regime, etc.) if needed.
22. May be required to perform tube feedings, ROM exercises, and administer medications if needed.
23. Report any noted changes in residents condition to physician and Director of Nursing as necessary.
24. Assists with passing of meal trays and resident eating as assigned.
25. Attends in-service programs when scheduled.
26. Attends staff meetings as assigned.
27. Attends and participates in daily stand-up QI meetings.
28. Contacts RN supervisor or RN on-call for tasks unable to perform as a Licensed Practical Nurse.
29. Establishes a good rapport with residents, families, and co-workers.
30. Encourages both licensed and non-licensed staff in constantly improving quality of nursing care.
31. Assists in admissions and discharges of residents, if needed.
32. Enters nurses notes in EMR, as necessary.
33. Is required to participate in licensed employee staffing on-call rotation.
34. Demonstrate competency using current clinical software system.
35. Adheres to No Lift Policy.
