Location: Bay Area (Sacramento, Stockton, Vallejo, Antioch, Alameda, Hayward, San Jose, Redwood City, Novato)

Position Overview:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in wound care to join our mobile wound care clinic. This role involves traveling to patients' homes to provide expert care for long-term, non-healing wounds. The ideal candidate will have experience in wound care, with a preference for those who hold a wound care certification. We are also open to supporting the right candidate in obtaining their wound care certification if they do not already have it.
Key Responsibilities:
- Provide expert wound care in patients' homes for non-healing wounds, including but not limited to:
1. Wounds that are too large to heal on their own
2. Burns
3. Skin loss from serious skin infections
4. Post-surgical wounds, including those from skin cancer surgeries
5. Venous stasis ulcers
6. Wounds following mastectomy or amputations
- Conduct thorough patient assessments to develop individualized treatment plans.
- Monitor wound healing progress and adjust treatment plans as necessary.
- Educate patients and their families on proper wound care techniques and preventive measures.
- Coordinate care with other healthcare providers, including primary care physicians and specialists, as needed.
- Maintain accurate and up-to-date patient records in accordance with HIPAA and other regulatory requirements.
- Participate in continuous education and training to stay current on best practices in wound care.
Qualifications:
- Education: Master’s or Doctoral degree in Nursing.
- Licensure: AB 890 preferred but not required, Current, unrestricted Nurse Practitioner license in the state of California.
- Certification: Wound Care Certification preferred (WCC, CWON, CWS, etc.). Willingness to obtain certification if not already certified; financial support for certification will be provided.
- Experience: Minimum of 2 years of experience as an NP, with a preference for experience in wound care.
- Skills:
o Strong clinical skills in wound care and patient assessment.
o Excellent communication and interpersonal skills.
o Ability to work independently and manage time effectively.
o Compassionate and patient-centered approach to care.
- Other Requirements:
o Valid driver’s license and reliable transportation to travel to patient homes.
o Ability to handle physically demanding tasks, including standing for long periods and performing wound care procedures.
