What are the responsibilities and job description for the Speech Language Pathologist (SLP) position at National Home Health Services?
National Home Health is an industry leader in post-acute home health care services. We are currently seeking a California Registered Speech Therapist who will be going to individual patient homes in order to provide treatment as ordered by the patient’s physician.
Immediate needs in the following areas: Santa Clara County and/or Peninsula as well as surrounding areas
Job duties include but are not limited to the following:
- Evaluates speech and language abilities and patient’s level of function.
- Plans and administers rehabilitative services for speech and language disorders.
- Assists in developing and implementing the patient plan of care.
- Observes, reports and records types of patient treatment as well as patient’s progress and/or regression.
- Submits written reports of patient progress at the designated intervals of every thirty (30) days and at discharge using the appropriate forms.
- Selects and administers diagnostic and therapeutic techniques and material for patient evaluation and treatment purposes.
- Instructs, teaches, supervises and counsels other applicable personnel and family members in methods of assisting the patient in improving, correction and/or accepting his or her disabilities.
- Instructs, teaches and supervises the patient’s family in the total speech therapy program.
- Participates in in-service education and staff development activities.
- Evaluates home environment and makes appropriate recommendations.
- Performs other duties as assigned.
Qualifications:
- ST must be a certified member of the American Speech and Hearing Association.
- Possess a Master’s degree in speech pathology or its equivalent
- At least one (1) year of supervised work experience with adult patients
- Licensed by the California Board of Speech-Language Pathology, Audiology and Hearing Aid Dispensers.
