">We are seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members.
">- Conduct comprehensive in-home Health Risk Assessments (HRAs)
- Review medical history, medications, lifestyle, and social determinants of health
- Perform physical and mental health evaluations and document findings in real-time
- Provide preventive care education without the need for ongoing patient management
