We are looking for Nurse Practitioners interested in a flexible side job. This per diem job allows you to customize your schedule and earn supplemental income while maintaining an autonomous relationship.
Responsibilities of the Nurse Practitioner - Health Risk Assessments:
- Conduct comprehensive in-home health risk assessments on Medicare Advantage members
- Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
- Educate and counsel patients on identified conditions and screening procedures
- Document findings using the provided iPad
- No continuum of care, prescribing, on-call, or follow-up is required
Requirements of the Nurse Practitioner - Health Risk Assessments:
- Active and unencumbered Nurse Practitioner license
- Minimum availability of 18 hours per week (including one full 8-hour day) is required to qualify
- Mileage is paid at $ .67 per mile, paid training, and no-show compensation is provided
- Travel Radius Flexible; more availability and more of a radius will get a priority
- Zip Code exclusion - Yes, you can exclude specific zip codes
- No contact with the patient's PCP - notes are sent electronically
- A collaborating Physician is assigned, and Malpractice insurance is provided
