Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components.
Job Description
Essential Functions:
- Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
- Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum.
- Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
- Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians.
- Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
- Develops clinical protocols, policies, and procedures including community education and public relations.
- Bachelor's Degree, required.
- Masters Degree in Nursing, preferred.
Registered nurse in the State of Ohio, required.

- Effective customer service skills.
- Good multiplatform communication skills (oral, written, presentation).
