Job Summary:

Responsibilities:
- Provide direct patient care to residents in a long-term care setting
- Administer medications and treatments as ordered by physicians
- Monitor patients' progress and report any changes to nurses and physicians
- Maintain accurate and detailed records of patient care
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
Requirements:
- Licenses: LPN/LVN license in the state of Illinois
- Certifications: Current CPR certification
- Experience: At least 6 months of experience as an LPN/LVN in a long-term care setting
- Education: Diploma or Associate's degree in nursing
