Position Summary:
The position is Grant funded position with a minimum of 3-year employment term (01/2025 – 06/2027). Positions may be extended as more funding is secured.
Under the direct supervision of the Produce Prescription Pilot Program (P4) Coordinator, this position will support, coordinate, and implement grant-funded initiatives awarded to the Navajo Health Foundation/Sage Memorial Hospital under the P4 program.
Program areas may include wellness, diabetes, obesity, nutrition, physical activity promotion, gardening, and other public health prevention programs.

Essential Duties & Responsibilities:
- Implement program activities in alignment with the P4 grant narrative and objectives.
- Contribute to program development and activities to meet grant objectives.
- Coordinate the purchasing and delivery of fresh produce from local grocers to target communities and participants of the Produce Delivery and Produce Voucher Program.
- Educate participants on nutrition and healthy lifestyle practices through workshops and educational sessions focused on the benefits of consuming a balanced meal, with emphasis on fruits and vegetables.
- Provide participants with health education materials and resources related to the P4 Produce Delivery or Voucher Programs.
- Build and maintain trusting relationships with patients and their families by providing support, encouragement, and guidance.
- Act as a patient advocate and liaison between participants and families with relevant support services.
- Coordinate and conduct follow-up with patients and families including basic motivational interviewing, goal setting, and regular contact via phone calls, home visits, or visits to other relevant settings.
- Monitor and support participant compliance with program requirements and track progress toward program goals.
- Collaborate with medical and clinical personnel to ensure cohesive care for participants.
- Remain knowledgeable about community resources to connect patients and families with relevant support services.
- Refer patients to community agencies or resources as appropriate to their needs.
- Foster strong working relationships with patients, visitors, staff, and program partners.
- Record patient care and program management information in the EHR and other software within 24 hours of contact.
- Attend staff meetings, training sessions, and other program related gatherings as requested.
- Maintain strict HIPAA compliance to always ensure the confidentiality of patient information.
- Evaluate program effectiveness and suggest improvements to enhance outcomes.
- Perform additional assigned duties to achieve the goals and objectives of the P4 program.
