Position Summary:

The Bereavement Coordinator is responsible for providing support to families who have experienced the loss of a loved one under the care of our organization. This includes overseeing bereavement assessments, managing the bereavement process, assisting with grief support, and coordinating various communication tasks such as check-in calls, mailings, and special events. The position requires attention to detail, compassion, and the ability to manage multiple tasks simultaneously.

Primary Responsibilities:

Huddle Email Management:

  • Review and track information related to patient deaths from daily huddle emails sent by the clinical team.
  • Identify patients who have passed and update the Excel report with relevant details such as patient’s name, date of death, gender, age, assigned social worker, and bereavement assessment status.

Excel Report Management:

  • Maintain the daily updated data on Excel spreadsheets for tracking:
    • Patient’s information (name, gender, age, team, etc.)
    • Bereavement Assessment due dates and statuses
    • Condolence card status
    • Bereaved family’s contact information (name, relationship, address, phone)
  • Highlight entries based on assigned social worker, risk level, or status (moderate risk, declined contact).

Bereavement Assessments (BA):

  • Monitor and track the completion of Bereavement Assessments for families.
  • Send weekly updates to the team (e.g., Support Services Manager, social workers) regarding overdue and upcoming assessments.
  • Complete Bereavement Assessments when a social worker has not been assigned, ensuring timely contact within two weeks of the patient’s death.
  • Schedule and document calls or in-person visits as per BA guidelines.

Condolence Cards:

  • Write and prepare condolence cards within two weeks of a patient’s passing.
  • Ensure the correct information is entered on the cards and gather signatures from the interdisciplinary team.
  • Track and follow up on the status of condolence cards (e.g., address information, missing signatures, etc.).
  • Mail condolence cards after ensuring proper documentation in both the Excel report and EMR system.

Check-In Calls:

  • 5 Week Check-In: Ensure calls are made to bereaved families five weeks after the death. If unable to reach, schedule a follow-up call.
  • 6 Month Check-In: Complete calls six months after the death, following the appropriate scripts and guidelines.
  • 13 Month Check-In: Ensure calls are made 13 months after the death and provide additional support as needed.
  • Maintain proper documentation of each call, including summaries, dates, and follow-up actions. Upload the documentation to the EMR system.

Mailings:

  • Prepare and send mailings for the 2-month, 6-month, and 13-month check-ins. This includes generating letters using mail merge, assembling grief education materials, and preparing mailing labels.
  • Mailings occur twice a month on the 1st and 15th, ensuring timely delivery and proper documentation in both Excel reports and the EMR.

Support Calls and Grief Group Assistance:

  • Take and return support calls from bereaved families, providing information about the bereavement program and scheduling 1:1 sessions as needed.
  • Co-facilitate grief support groups, assist in developing group topics, and track group attendance and documentation.

Grief Boxes:

  • Assemble grief boxes as needed, ensuring they are provided to families during Bereavement Assessments or as part of ongoing support.

Special Events:

  • Assist in planning, developing, and executing special events related to bereavement, such as memorial services or other family support activities.

Documentation and Reporting:

  • Maintain accurate and detailed records of all interactions, assessments, and communications with bereaved families in the EMR system.
  • Ensure all guideline tasks are entered into the EMR and Excel report, with proper notes and comments regarding completion and follow-up actions.

Knowledge & Skills Requirement:

  • Strong organizational skills and attention to detail.
  • Excellent written and verbal communication skills, with a compassionate and empathetic approach.
  • Ability to manage multiple tasks and deadlines simultaneously.
  • Familiarity with medical records systems (EMR) and Excel.
  • Ability to work collaboratively with interdisciplinary teams.
  • Experience in grief counseling or support services is preferred.
  • Comfort with phone and in-person communication with grieving individuals and families.
  • High level of professionalism and confidentiality.

Working Conditions:

  • Indoors in an air-conditioned office, in patients/families homes, hospitals, nursing home facilities as well as outdoors in all types of weather.
  • Occasional travel for in-person visits or special events may be required.

All offers are contingent on a candidate's successful completion of the following requirements:

  • A legally compliant background check
  • Drug Screen & Physical Exam
  • TB Clearance
  • Use of personal motor vehicle which meets valid registration and insurance requirements. Must possess a valid Drivers' License, current auto insurance, and reliable transportation. Must have acceptable driver's abstract.

Hours:

  • Generally, Monday through Friday, 8:00 am through 4:30 pm. May require flexible hours depending on the needs of the families and scheduled events.

$65,000 To $70,000 Per Year
