Bereavement Counselor

Position Summary:

The Bereavement Counselor plays a critical role in delivering high-quality bereavement services to the families of our hospice patients. The Bereavement Counselor under the supervision of the Support Services Manager will work closely with the Interdisciplinary Team at Navian Hawaii to provide holistic grief support. This role ensures that all bereavement services align with the philosophy, goals, and objectives of hospice care while fostering the professional development of team members. The Bereavement Counselor is dedicated to helping families navigate the grief process and ensuring that services are compassionate, timely, and effective.

Primary Responsibilities:

Bereavement Counseling & Support:

• Provide one-on-one counseling and complicated grief follow-up for surviving family members in accordance with Navian Hawaii's guidelines.
• Develop and implement individualized bereavement plans of care for families, ensuring all documentation meets established standards and guidelines.
• Collaborate with the Interdisciplinary Team to identify and address bereavement risks, including reviewing complicated grief cases and providing targeted support.
• Collaborate with Navian Hawaii social workers to ensure that families receive the appropriate support during the bereavement process.
• Facilitate bereavement support groups to foster healing and provide ongoing support.

Program Management:

• Manage the logistics of condolence card mailings, follow-up calls, and other related communications, ensuring timely and compassionate outreach.
• Maintain and update tracking spreadsheets to monitor the delivery of bereavement services and evaluate outcomes.
• Oversee the distribution of surveys following the completion of 13 bereavement care sessions with grieving families, analyze the results, and develop strategies to enhance the bereavement program.
• Participate in regular meetings with the Support Services Manager to assess departmental progress, review goals, and discuss ongoing initiatives.
• Prepare and submit reports to the Director of Clinical Operations as needed.
• Actively participate in Bereavement Department and Support Services meetings, sharing insights, reviewing progress, and ensuring effective service coordination.

Community Support & Involvement:

• Plan, prepare, and facilitate Navian Hawaii’s annual Memorial Service and other bereavement-related events.
• Provide educational programs and materials for staff and the larger community on topics of grief, loss, and bereavement.
• Collaborate with Social Workers to conduct thorough bereavement assessments and identify the needs of families.
• Respond to community bereavement requests, including crisis outreach, school and workplace grief support, and community grief education.
• Support the Volunteer Department in the development and delivery of bereavement-related volunteer training and services.

Miscellaneous Duties:

• Perform other duties and projects as assigned by the Support Services Manager, Director of Clinical Operations or the President of Navian Hawaii.

Knowledge & Skills Requirements:

• In-depth knowledge of grief counseling, bereavement support, and ethical guidelines relevant to the field.
• Strong understanding of the Medicare Conditions of Participation and how they apply to hospice care.
• Proficiency in assessment, treatment planning, case management, and psychotherapy techniques.
• Comprehensive understanding of individual, group, and family therapy methods, as well as social and emotional impacts of health issues on patients and families.
• Ability to develop and implement treatment plans that are tailored to the specific needs of patients and families.
• Ability to communicate effectively with a wide range of individuals, including staff, clients, and external agencies.
• Strong organizational skills to manage multiple tasks, programs, and records efficiently.
• Skilled in using Microsoft Office (Word, Excel, Outlook, Teams) and electronic medical record software.

Experience & Educational Requirements:

• Professional Clinical Degree from an accredited institution.
• Bachelor’s degree or higher preferred.
• At least two years of direct experience in grief counseling, ideally within a hospice or related healthcare setting.

Hours:

• Generally Monday to Friday, 8:00 AM to 4:30 PM.
• Must be available for after-hours work, including weekends and holidays, as needed.

Salary Range:

$65,000 To $70,000 Per Year
