Positions Summary:
The Intake Lead is responsible for overseeing all aspects of the patient intake process and managing the referral team for our non-profit hospice organization. This role is crucial in ensuring a smooth transition for patients and families seeking palliative and hospice care, while maintaining positive relationships with referral sources and healthcare partners. Primary Responsibilities:
Team Management
- Oversee and lead the intake team, including both clinical and non-clinical members
- Provide guidance, training, and support to team members to ensure efficient and compassionate service
- Establish performance metrics and conduct regular evaluations to maintain high-quality standards
- Serve as the primary liaison between the intake team, clinical staff, community liaisons, and external partners
- Facilitate effective communication among all stakeholders involved in the patient admission process
- Provide regular updates to leadership on intake and referral performance metrics
Intake Process Oversight
- Manage all aspects of the patient intake process, from initial contact to admission
- Develop and implement strategies to streamline the intake workflow and improve efficiency
- Ensure compliance with organizational policies, procedures, and applicable laws and regulations
Referral Management
- Establish and maintain positive relationships with referral sources, including hospitals, physicians, and healthcare facilities
- Collaborate with Admissions Manager and Director of Business Development to develop and implement referral growth strategies
- Monitor referral trends and provide regular reports to leadership
Quality Assurance
- Implement and maintain quality assurance processes for intake and referral procedures
- Conduct regular audits to ensure accuracy and completeness of intake documentation
- Address any issues or discrepancies promptly and effectively
Experience & Education Requirements
- Bachelors degree in healthcare administration, Nursing, or related field; Master's degree preferred
- Minimum of 5 years of experience in hospice or healthcare management, with at least 2 years in a supervisory role
- Strong understanding of hospice eligibility criteria, Medicare/Medicaid regulations, and insurance processes
- Excellent interpersonal and communication skills, with the ability to interact compassionately with patients, families, and healthcare professionals
- Proven leadership abilities and experience in team management
- Proficiency in electronic medical record (EMR) systems and data analysis tools

- Use of personal motor vehicle which meets valid registration and insurance requirements. Must possess a valid Drivers' License, current auto insurance, and reliable transportation. Must have acceptable driver's abstract.
Hours:
- Generally, Monday through Friday, 8:00 am through 4:30 pm, with occasional evening or weekend work as required
