Scheduling Specialist- Home Health Care- Monday through Friday

We are currently seeking a detail-oriented and driven Full-Time Scheduling Specialist to assist in the coordination of patient care, under the direction of the Executive Director, while fostering Navigate Home Health Care's Mission and Philosophy.
Responsibilities:
- Coordinate patient care across various healthcare disciplines.
- Maintain and update a current patient roster with necessary information.
- Manage and adjust schedules as needed to accommodate changes in patient care.
- Obtain daily to meet weekly productivity requirements and report results to the Clinical Manager.
- Facilitate care transitions by sending transfer and discharge summaries to the next care providers.
- Ensure timely submission of medical orders and track their return or arrange for hand carry by Account Executives as necessary.
- Fax out notifications of missed visits daily to the corresponding ordering practitioner to ensure effective care coordination and compliance with healthcare regulations.
- Report to Clinical Manager all clinical issues and grievances reported by patients or staff.
- Act as a liaison between patients, field staff, and office.
- Perform all coordinating duties related to scheduling, when necessary.
- Directly support the Clinical Manager by relaying critical information to field staff, office staff, and community liaisons.
- Ensure scheduling of staff is done with consideration to their skill sets, avoiding assignment of clinicians to clinical duties they are unprepared for or have never performed, ensuring all clinicians are properly matched to patient needs requiring specific clinical duties.
- Manage contingent staffing needs and document all personnel changes.
- Handle administrative matters related to the care team and the patients they serve.
- Communicate, on a routine basis, with contract agencies, to maintain smooth relations and to obtain needed information.
- Collaborate with the intake to track pending authorization requests, ensuring all necessary documentation and approvals are obtained promptly to facilitate uninterrupted patient care.
- Maintain a scheduling tracker using Microsoft Excel.
- Reports to be addressed daily: Schedule Deviation, Visit Exceptions, Orphan Documents, Therapy Re-Evaluation Exception, missed visits, Therapy evals over 48 hours. Orders preventing billing or over 12 days.
- Execute additional duties as assigned by the Supervisor, Clinical Manager, or Administrator.
Qualifications:
- High school diploma or equivalent required; Associate degree in healthcare administration or related field preferred.
- Minimum of 1 year of experience in a scheduling or similar role, preferably within a home health or healthcare setting.
- Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
Job Conditions:
- The position may involve high-stress levels due to strict deadlines that must be met consistently. Candidates should be prepared to handle pressure and manage time efficiently.
- This role is primarily sedentary, requiring extended periods of sitting at a desk. It also involves some standing, stooping, and walking. The nature of the job demands a significant amount of telephone communication, necessitating comfort and efficiency in this mode of interaction.
- Minimal lifting is involved, primarily concerning office records and printouts. The job does not demand extensive physical exertion but does require basic handling of office materials.
- Candidates must have the ability to read type of 12 points or larger. Adequate hearing is necessary for effective telephone communication. The use of an amplifier is permitted to fulfill job functions. Clear verbal and written communication skills in English are essential for successful performance in this role.
