Care Transition Nurse
- Responsibilities : As a Care Transition Nurse, you will assist patients through various stages of care, ensuring a seamless transition between healthcare providers, settings, and treatment phases. You will assess patients' needs, educate them on self-care, coordinate discharge plans, and ensure they understand their care instructions and follow-up appointments. You will work closely with doctors, social workers, and insurance companies to facilitate a smooth transition from hospital to home or other care facilities.
- Required Skills : Expertise in care transitions, patient education, discharge planning, and managing multidisciplinary care teams.
- Qualifications : Registered Nurse (RN) with experience in patient navigation, care transitions, or case management.
- Stipend : Total stipend of $2,700 weekly / $11,550 monthly.
