Home Infusion Pharmacist

Qualifications :

B.S. in Pharmacy or PharmD.
NYS licensed; prior experience in home infusion or specialty pharmacy preferred.
Expertise in sterile compounding and intravenous therapies.
Must pass pre-employment health clearance.

Duties :

Prepares and dispenses IV medications for home-based patients.

Educates patients and caregivers on proper infusion techniques.

Coordinates with home healthcare providers for seamless treatment.

Ensures sterility and safety of compounded infusion therapies.

Maintains compliance with regulatory and institutional policies.
