Qualifications :

L.M.S.W., L.C.S.W., or eligibility for licensure
Minimum of three years of experience in mental health, crisis intervention, or inpatient care

Duties :

Assess and support patients facing acute mental health challenges

Develop discharge and transition care plans, ensuring access to follow-up services

Provide counseling, crisis support, and community resource navigation

Collaborate with healthcare providers to enhance patient-centered care

Additional responsibilities as assigned

Pay : $60 - $65 / hourly

Salary : $60 - $65
