Job Title: Patient Adherence Specialist - Medical Support
Duties:
- Facilitate patient attendance at medical appointments and provide transportation assistance as needed.
- Ensure patients adhere to medication and care plans, addressing any barriers.
- Educate and coach patients on health management strategies.
- Liaise with healthcare providers to monitor and promote treatment adherence.
- Manage a caseload of up to 20 patients, offering weekly or monthly health education.
- Conduct reassessments of entitlements and benefits eligibility.
- Coordinate logistics for patient adherence, including appointment reminders and childcare.
- Document encounters and input data into online systems, including grant reporting tasks.
Skills Required:
- Strong written and verbal communication skills.
- Proficiency in Spanish is a plus.
- Attention to detail with a focus on timely and accurate work.
Pay Range:
- Weekly: $1,250 - $1,500
