Patient Care Coordinator Nurse
- Responsibilities : As a Patient Care Coordinator Nurse, you will provide personalized care and support to patients throughout their healthcare journey. You will help patients understand their diagnosis, treatment options, and navigate through the healthcare system. Your responsibilities include coordinating appointments, managing communication between the patient and healthcare providers, ensuring timely follow-ups, and offering guidance on managing their health. You will also assist patients with insurance and referral processes.
- Required Skills : Strong organizational skills, patient education, excellent communication, ability to coordinate care across multiple providers, and knowledge of healthcare systems and insurance processes.
- Qualifications : Registered Nurse (RN) with experience in patient coordination, case management, or patient navigation.
- Stipend : Total stipend of $2,500 weekly / $10,500 monthly.
