Job Title: Patient Care Services Liaison

Duties:

  • Facilitate care plans in collaboration with Primary Care Providers and clinical teams.
  • Organize and manage follow-up appointments, referrals, and pre-certifications.
  • Address patient concerns and guide them through available healthcare resources.
  • Maintain care dashboards to ensure timely task management and appointment coordination.
  • Communicate effectively with patients and insurance carriers to resolve issues.
  • Recommend process improvements to streamline workflows and enhance patient outcomes.

Skills Required:

  • 4 years of healthcare, customer service, or administrative experience.
  • Excellent interpersonal and analytical skills.

Education:

  • Associate’s degree required; Bachelor’s degree preferred.
  • Knowledge of medical terminology is beneficial.

Pay Range:

  • Weekly: $1,325 - $1,600
