Job Title: Patient Support Navigator - Care Coordination
Duties:
- Assist patients with attending medical appointments and provide accompaniment when needed.
- Address medication adherence challenges, ensuring patients follow prescribed regimens.
- Deliver tailored education and coaching to meet specific program requirements.
- Serve as the primary point of contact for medical providers to support patient care plans.
- Maintain a caseload of up to 20 patients, providing ongoing health education and support.
- Conduct entitlement reassessments and coordinate necessary logistics, including transportation and childcare.
- Document all interactions in a web-based system and support grant-related data entry tasks.
- Perform additional duties as required.
Skills Required:
- Strong communication skills (written and oral).
- Spanish language skills preferred.
- High attention to detail and accuracy in documentation.
Pay Range:
- Weekly: $1,250 - $1,500
