We are seeking an experienced RN - Case Manager with a strong background in acute care hospital case management and proficiency in EPIC Electronic Medical Records (EMR). Candidates with Emergency Department Case Management (ED CM) experience are preferred.
Qualifications:
Active RN License in the state of New York
BLS Certification (must be certified or able to recertify)
Minimum 1.5 years of recent experience (within the past 2 years) in an acute care facility of similar size and level
Experience in hospitals of equal or greater size or trauma centers is required for travelers
Strong interpersonal and communication skills
Preferred Experience:
Emergency Department Case Management (ED CM) experience
Key Responsibilities:
Perform utilization review, case management, and clinical documentation improvement (CDI)
Collaborate with interdisciplinary teams to ensure effective patient care and resource utilization
Ensure accurate and thorough documentation in EPIC EMR
Advocate for patients while maintaining compliance with hospital policies and regulatory standards
