RN - Maternal Health Case Manager
Responsibilities :
- Provide case management and nursing care for clients during the prenatal and postpartum periods.
- Conduct screenings for social determinants of health (SDOH), perinatal depression, and pregnancy / postpartum risk factors.
- Develop individualized care plans and measurable goals with clients to monitor progress.
- Educate clients on perinatal health standards and provide short-term interventions and emotional support.
- Complete postpartum home visits within 30 days of delivery as needed.
- Document all interactions and interventions in the electronic medical record (EMR) within one business day.
- Attend required virtual training sessions, meetings, and workshops as directed by the supervisor.
Qualifications :
Proficiency in Microsoft Office and experience with EMR systems, preferably Epic.
