Position Type: Contract
NC Health Partners (NCHP) is currently looking to hire a Qualified Professional (QP) to work with youth in the community between the ages of 7 and 17 on the Intensive In-Home (IIH) team. This candidate should have at least 2 years of experience serving children and adolescents with mental health and/or behavioral health diagnosis.
Summary of Position Responsibilities:
Under the supervision of the Team Lead, the QP works with the client to achieve their treatment goals. The QP will provide therapeutic sessions to consumers and families based on identified needs. The QP is responsible for maintaining a treatment team caseload between 6-12 as well as completing Person-Centered Plans (PCPs) updates of for consumers being re-evaluated for services.
Essential Duties:
- Helps consumer achieve goals as indicated by the PCP
- Assists with crisis interventions
- Assists the Team Lead with behavioral interventions
- Consults with identified medical (i.e., primary care and psychiatric) and non-medical providers, engages community and natural support and includes their input in the person-centered planning process
- Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
- Monitors and documents the status of the beneficiary’s progress and the effectiveness of the strategies and interventions outlined in the PCP
Qualifications and Education Requirements:
- Associate Degree or higher
- Able to successfully pass a criminal background check
- Clear Health Care Registry
- At least 2 years of experience working with youth who have a history of behavioral and mental health diagnosis.

Job Type: Contract
Pay: $15.00 - $20.00 per hour
Schedule:
- Monday to Friday
- Weekends as needed
