Job Summary:
The Social Worker is responsible for enrolling, providing education and support to individuals in need of prescription medication assistance. They will work with patients and/or providers to determine eligibility for the Free Pharmacy Program and direct them to appropriate support services, if applicable. This position reports to the Director of Patient Engagement, but will work closely with the pharmacy and partner relations teams in the execution of their duties.
Responsibilities:
- Educate potential patients about the Free Pharmacy Program (FPP) eligibility requirements
- Screen patients for FPP eligibility and make referrals to external resources, as needed.
- Assist patients and/or providers in obtain the necessary documents for FPP enrollment.
- Accurately enter patient information into the database.
- Communicate approvals and denials with patients and clinic partners in a timely manner.
- Provide follow up throughout the application process until the enrollment application is approved; i.e. make and document follow-up attempts to patient and/or clinic partner with incomplete applications.
- Treat all patients and clinic partners with respect and dignity. Provide customer service excellence and always work calmly, professionally and offer compassion even under stressful conditions.
- Communicate frequently with the Director of Patient Engagement when patient or clinic is dissatisfied with application decision or with general service complaints.
- Communicate, as needed, with the Director of Partner Relations when clinic partners fail to provide correct documentation.
- Update patient information in a timely manner; i.e. change of address or recertification as soon as it is received.
- Maintain up-to-date knowledge of the medication/prescription assistance programs and refer ineligible patients to appropriate resources.
- Assist with building awareness of the program mission by speaking to groups, attending health fairs, and other duties as assigned by the Director of Patient Engagement.
- Assist with Mobile Free Pharmacy Program events as needed (overnight travel may be needed.
Qualifications:
- Associate or Bachelor’s degree or combination of relevant work experience. Background in health, human services, community health or nonprofit, preferred.
- Able to demonstrate customer service excellence.
- Excellent computer skills with abilities in Word, Excel, PowerPoint, etc.
- Comfortable working in an in-person and remote environment.
- Excellent verbal, written, and communication skills.
- Excellent organization skills.
- Must be willing and have experience in participating as part of a team and establishing working relationships that foster optimal quality of productivity.
- Demonstrates ability to effectively interact with people of diverse socioeconomic, cultural, disability, and ethnic backgrounds.
- An outgoing and engaging personality with confidence to interact face-to-face with individual and community stakeholders; Provide customer service excellence even under stressful conditions
- Must have a valid driver’s license and reliable transportation.
- Must be able to lift 25-30 lbs. and able step into and out of NC MedAssist van/truck unassisted.
- Proficient and comfortable reading, writing and speaking in Spanish on a daily basis.
Compensation
Compensation range is hourly and based on the candidate’s qualifications, experience and skills.

Physical Requirements (with or without reasonable accommodations)
- Availability to work flexible hours including some weekends and evenings
- Comfortable with changes in the environment with the ability to work in a sedentary office environment for extended periods of time.
