What are the responsibilities and job description for the Support Coordinator/Case Manager position at NCA 310?
Job Description: NOTE: $16.00 per hour. This is a responsible professional position in the area of providing support coordination to individuals with Developmental Disabilities.
DUTIES:
Duties shall include (but are not limited to):
- Possess the ability to obtain, establish, and compile background information on individuals with developmental disabilities
- Coordinate individuals to other agencies for services as needed
- Verify eligibility under appropriate guidelines
- Write social assessments or coordinate this function with other support coordinators.
- Complete and submit necessary reports, records, etc. as required by the funding agency including home visits and communications with families
- Perform other appropriate duties assigned by the Director in cooperation with other staff
REQUIRED KNOWLEDGE AND SKILLS:
The Support Coordinator shall possess:
- Ability to organize work and comply with the requirements of the Alabama Department of Mental Health, Developmental Disabilities and Medicaid
- Ability to establish rapport with participants and work cooperatively with parents/guardians, staff, and other agencies.
- Ability to express self effectively, orally, and in writing.
- Ability to understand and carry out complex oral and written instructions.
- A valid Alabama driver’s license and a car to be used in performance of job duties as needed.
- Be able to perform duties including case management and billing for Medicaid on a computer.
QUALIFICATIONS:
Must possess a Bachelor’s degree in the Human Services field. Prefer at least one year employment as a Support Coordinator in a setting serving individuals with developmental disabilities where he/she was required to formulate decision based on individuals needs.
