In this role, you will deliver intensive home-based treatment services to you facing significant emotional and behavioral challenges. Your primary focus will be empowering youth and caregivers to develop robust coping mechanisms, preventing the need for out-of-home placement, or aiding in a seamless transition back home from a more intensive care setting.
Intensive Home-Based Treatment Services & Case Load Management
- Deliver behavioral health services by assessing clients’ therapeutic needs and strengths, establishing therapeutic relationships, and employing interventions to enhance overall functioning
- Act as a central point of contact for families, agencies, and court, maintaining communication within Necco’s confidentiality policies and procedures regarding clients’ performances
- Take on the role of clinical lead for cases, developing, writing, and coordinating individual treatment plans. Responsibilities encompass reviewing external materials, discussing client progress with staff, revising plans for optimal care, monitoring plan compliance, participating in team meetings, and attending group meetings to discuss treatment
- Implement best practice treatment models to ensure effective intervention strategies and Individual and Family Therapies, Therapeutic Behavioral Services (TBS), and/or community psychiatric supportive treatment, tailoring interventions to meet client requirements
- Consult with parents/caregivers, school personnel, probation officers, or other treatment team members to address client’s diagnosis, progress and barriers as needed.
- Maintain prior authorizations for each client (if applicable)
- Maintain caseload of no less than 3- IHBT clients but not to exceed 6-IHBT clients at any given time
- Drive the treatment planning process from intake to discharge based on client diagnosis and need
- Meets social impact expectations
- Maintain complete and accurate client records per regulations
- Ensure treatment goals are tied to IHBT outcomes
- Timely submission of all documentation within three calendar days of service delivery
- Adherence to Ohio Medicaid documentation standards of proactive to ensure the integrity of all documentation
- Sustain ongoing compliance with client clinical chart requirements
- Meet Necco standards and productivity through comprehensive documentation, including assessments, progress notes, reports, and Individual Service Plans
- Practice ruthless pragmatism

- Bachelor Degree in Social Work (LSW), or Masters Degree in Social Work, Counseling, or Marriage and Family Therapy with at least dependent license (LSW, LPC, or LMFT)
- Hold a current Certified CANS Assessor credential or is able and willing to become a Certified CANS Assessor
- Demonstrate the ability to visit clients' homes, recognizing that accessibility may vary
- Work effectively with culturally diverse populations and families
- Fulfill additional duties as required
- Valid driver’s license with the ability to drive during day and night
- Reliable transportation
- Successful completion of all criminal and background checks
- Must possess the skills to navigate and execute software programs used by Necco (i.e. Microsoft Office, Electronic Health record, and Dashboard)
